What  patient should know & follow when considering surgery

Patients posted for major surgery under anaesthesia by an anesthesiologist or minor procedure under local anaesthesia or MAC should follow some basic things. They should tell about any illness even minor or chronic to their doctors. The disease may be under control due to medications or diet or otherwise like Diabetes, Hypertension, but the fact must be informed to the consulting doctors. The patient must tell about any medications they are taking including Ayurveda, Homoeo, Unani, Sidha etc. also to the doctor.  The doctors may advise change of drugs, advise dose adjustment or advise further investigations and also may change their plan of anaesthesia or surgery may change depend on these. The patient must tell about any previous surgery, implants etc.
When visiting the doctor have a responsible adult bystander with you always and it is preferable that he is your close relative like spouse, parents, children or siblings and be with you in al visits and perioperative periods so that communication between the doctor and the patients & his relatives becomes easy. The cell phones must be kept Off at places where electronic medical equipments are kept connected to the patient.
Pre Operative starving is a must. The patient must not take any food for 6 hours prior to surgery. However clear liquids like black tea or coffee, clear juice can be taken upto 2 hr.s prior to surgery.  Patient should take good bath before to changing to Theatre dress and wear clean theatre dress after removing all his clothing and ornaments, removable implants etc. 
More important is, if you have any doubts, clear them with your doctor. Follow your doctors’ advises strictly.  Help doctor to serve you better. Never see him with doubt. He is always looking for your well being. The treatment plan including anaesthesia, investigation etc. is different for different patients even if the disease is same depending on the patient variables like age, sex, concurrent medical illness, medications, financial position etc.  He will always advise the best for you.  Have a speedy recovery.

CHILD CARE

Child health has remained an essential component of most of the National Programes in our country. Even though interventions to manage the common health events are available, new strategies have to be introduced to reduce the child mortality and to improve the child health and development. Improved management of common childhood illness as well as prevention of diseases and promotion of healthy dealing with counseling on feeding and immunization has to be adopted and strategies for neo natal care at home have to be introduced. Improvement in the family and community health care practices has to be done. Effective communication with the mother or care provider helps to re assure the mother or caretaker that the child will receive appropriate care. Success of home treatment depends on how well the mother or caretaker knows about giving the treatment and understandings its importance. A comprehensive approach to child health with the help of Panchayat Raj institutions for the basic education, nutrition, health care and sanitation available for proper development of every child in our place should be our aim in future.

According to Lamberston supervision is defined as a cooperative relationship between leader and one or more person to accomplish a particular purpose. According to Jean Barrett, supervision is a kind of teaching which involves advising, helping, inspiring, leading and liberating.
Supervision is an art of motivating personnel to do their best.
 The competency and attitudes of the personnel depend largely on the quality of guidance and supervision provided for the worker.
Good supervision generates and guaranties quality of work and co-ordinates the effort of staff. It is based on the individual needs, fosters the ability of each member to think and act and respects the individuality. Good supervision is well planned, stimulates staff to continuous self improvement and analyses the success of reaching one’s objectives. . Good supervision helps to create a social, psychological and physical atmosphere. Good supervision directs the self evaluation.
Supervision is a management activity. A good supervisor should be competent in the following managerial elements: Setting objectives, Forecasting, Planning, Organizing, Co-ordinating, Motivating, Controlling, and Monitoring.
Good supervisory skills would mean good results from the team.
A supervisor must be a role model. The following are some of the Qualities of a Good Supervisor : Punctuality ,effective communication skills, Thoroughness, Fairness ,Initiative ,Tact ,Enthusiasm ,Emotional control ,Personal Qualification ,Teaching ability , Sense of humour, Ability to motivate/inspiring their work, Delegating job effectively, Creating a no fear environment,  Focussing on their strength, Spending time with them, Impartiality ,Comprehensiveness, Sympathy, Mutual respect, Persuasive approach, Flexibility, etc.




Public health/Disease control: Each must do his duty

        The rising toll on human life and health by emerging and re emerging infectious diseases is the price for willful failure in keeping our living space CLEAN AND DRY- which is the key word in vector control. Primarily this is the duty of LSG because they are equipped and empowered for that. They should not shrug off the responsibility of maintaining environmental sanitation.
         May I suggest my friends in HSD to wake up and read Public Health Act. They must acknowledge the fact that they are the authority to invoke the Act and safe guard public health, duty bound. Sadly the PH Act in force in our state is a joke for PH offenders. It only barkes. It is high time the Government re enact a comprehensive PH Act that will bite and deter the offenders-including those erring LSGs. Some were there must be a penal provision as well  against Health Authorities who fail to invoke the Act when warranted, as has been happening over years.
